    • EMPLOYEE GOVT I CARD/APPOINTMENT LETTER/EMPLOYMENT PROOF.
    • EMPLOYEE ADHAAR CARD,PAN CARD,PASSPORT PHOTO.
    • DEPENDANT ADHAAR CARD,PAN CARD/BIRTH CERTIFICATE, PHOTO.
    • NOMINEE BANK ACCOUNT NUMBER AND PASSBOOK.
    • FAMILY PHOTO WITH EMPLOYEE AND DEPENDANT.
    • (Details of Aadhaar, Contact number, mail id, residential address, designation, PAN Number, department ( last designation and department in case of pensioners), dependent details, Bank account details of self and nominee, PPO details and PPO number( in case of pensioners) are to be uploaded in case of registration in the portal.)
  • Upload any required documents, such as identification proof, employment proof, or pensioner certificate.

5. Submit the Application:

  • After filling out the form and attaching the necessary documents, review your application to ensure all details are correct.
